Do you take insurance?
I currently accept Cigna, Blue Cross Blue Shield, Anthem, Aetna, and United Health Care through Fay Nutrition. If you have a plan through any of the above, book your appointment through Fay to pay little to nothing for my services (woo!). When you follow the link in the scheduling section, you will also be able to get a payment estimate.
If you have a different insurance plan, I am happy to provide a Superbill, which you may give to your insurance company to ask for reimbursement.
Will my visit be in person or virtual?
I am currently located in Fussa, Japan due to my husband's job in the Air Force. If you live on or near Yokota AFB, my preference is to conduct sessions in person at the Yujo Community Center, but I am flexible if this does not feel like an option for you.
If you do not live in Japan, your appointment will be virtual. I recommend booking a quick 15-minute call in order to ensure that my services are right for you. Since licensure laws are different state to state, this also helps me to double check that we can work together. If we are unable to work together, I'll give you some recommendations of dietitians in your area.
